LPN - Transfer/Admissions Coordinator

Looking for a responsible and detail oriented person who is a good communicator to be the first point contact between outside facilities and our hospitalist group for transfer and admissions of patients to our hospital.  You will need triage skills to assure that Grandview has the appropriate services needed by the patient as well as assessing the condition of the patient for transfer and determining the level of care required to care for the patient

Responsibilities

•           Answering incoming calls promptly and greet callers in a friendly, professional manner
•           Determining appropriate level of care for patients
•           Discussing need for transfer and availability of services
•           Interacting with bed control and house supervisor as needed to assure bed placement
•           Maintaining Hospitalist share drive with incoming admissions
•           Entering patients data into group billing system
•           Working with physicians and APPs to assure seamless transition of care
•           Working with case management for patient’s discharge needs

Location & commitments

•           Hours: 8:00am-5pm Monday through Friday
•           Grandview Medical Center Hospitalist Office

Candidate requirements

•           Licensed Practical Nurse
•           Strong multitasking skills
•           Good verbal and written communication skills
•           Must be able to speak fluently and clearly
